Screening programme overview

– The national screening program for breast cancer has been operating since 1992.

– The program’s target population consists of women aged 50–69. In 2016, full coverage in all age groups was achieved. Additionally, gender minority individuals at risk of breast cancer are eligible for screening. The screening method is mammography and the screening interval is two years.

– 344 851 persons were picked from the Digital and Population Data Services Agency database for screening invitation as of year 2024. The invitation can be left unsent, for example due to non-disclosure for personal safety reasons or missing address details. The Åland Islands are excluded from the 2024 screening statistics.

– In 2024, 82 % of invitees participated in the screening program. Of attendees, 2.9 % were invited to diagnostic examinations.

– Within the screening program, a total of 1904 breast cancer cases were found.

Data

• The data includes all persons invited to screening, and their screening related outcomes reported by spring 2026.
• The data on diagnoses only includes data from the screening registry; cancer registry data and data on diagnoses outside of the national screening program have not been linked to obtain these figures. For cancer detection mode classification, cancer registry data has been utilized.
  
• The relatively large proportion of invitation to diagnostic examinations for the age group of 50 years, compared to other age groups, is due to the lack of earlier mammograms for comparison.

Indicator definitions

• The participation rate was calculated by dividing the number of attendees with the number of persons invited.
• Screening findings are classified by screening round, according to the person’s most severe finding during the round.
• The confidence intervals model the uncertainty related to the size of the population and random variation. The intervals are calculated using the exact binomial model at 95% confidence.

Population classification

• Statistics Finland’s individual-level data on education were used in the production of statistics under permit TK/2743/07.03.00/2023.
• Education categories were created by combining the categories of pre-primary, lower primary, and upper primary education into the primary education category, combining secondary education and special vocational qualifications into the secondary education category, and combining lower and upper tertiary education and doctoral education into the tertiary education category. Unknown and missing data were combined with the primary education category. The education data is from the year preceding the screening year (2023).
• Native language and country of birth is based on data from the Digital and Population Data Services Agency.

Result classification

• Normal screening result: The next invitation will be sent in two years, if the person still belongs to the screening target population.
  
• Invitation to diagnostic examinations: The person was invited to supplementary diagnostic examinations (additional mammography, ultrasound, biopsy).
  
• Referral to specialized healthcare: The person was referred to specialized care for confirmation of diagnosis and treatment.
  
• Benign: Findings classified as benign based on SNOMED/ICD-O-3 morphology codes (behavior codes 0, 1 or 5).
• Malignant: Histologically confirmed invasive breast cancer or carcinoma in situ (DCIS, LCIS), defined using SNOMED/ICD-O-3 morphology codes (behavior codes 2 or 3), or cases without surgical histology but with a TNM stage of at least I.
• Other cancer: Histologically confirmed cancer of another organ detected in breast cancer screening (based on SNOMED/ICD-O-3 coding).
• No information: No histological data is available for the person referred to specialized care.

Cancer detection mode

• The cancer detection mode categories are defined as follows:
  
• Screened cancer: After the previous screening mammography, an invitation to diagnostic examinations was made and a biopsy was taken within one year prior to diagnosis.
  
• Interval cancer:
  • After previous screening mammography, neither an invitation to diagnostic examinations was made or a biopsy taken within 2.5 years prior to diagnosis.
    
  • Invitation to diagnostic examinations was made and biopsy was taken within 2.5 years prior to diagnosis, but the date of diagnosis was 1-2.5 years after the screening mammography.
    
• Unattended: The person was invited to screening within 2.5 years prior to diagnosis but did not attend.
  
• Not invited: The person was of screening age but had no registered invitation within 2.5 years prior to diagnosis.
  
• Younger than screening age: Diagnosis was made before the individual reached screening age.
  
• Older than screening age: Diagnosis was made more than 2.5 years after exceeding the upper age limit for screening.
